After evaluating a teacher it is necessary to meet with the teacher to go over the evaluation. The post evaluation form provides documentation that you did meet with the teacher. The following is a sample post evaluation form.
This is to verify that ________________________________ (Teacher’s Name) has received his/her post-conference related to the evaluation process from Any Where Public School District. This does not mean that I necessarily agree with the evaluation itself. However I understand what my administrator's expectations are and will take the suggestions, tips, and advice given to me back to my classroom to try and apply it.
